The Process

Our physicians require that each patient have current medical records that document the existence of one of the conditions similar to RCW 69.51a which  is authorized under their state law. You must either obtain your medical records yourself (which may involve a fee) and bring them in to us, or you may authorize that your medical records be faxed or mailed to one of our offices.

Three Key Steps to Setting an Appointment: Before you are ready!

Here is the Cover sheet for your doctor to fax to us. Download it and take it to your doctor.

Step 1: Go to the Doctor-If you are a NEW PATIENT, in order to schedule an appointment you must first have:

A) CHART NOTES (The notes your doctor keeps in your medical file.) The records need to be current within the past three years and must pertain to your qualifying condition. We only need chart notes from 2-3 recent visits on your condition. These can be UP TO three years old. For example, if you saw the doctor in September of 2006, those records could be used until September of 2009. Please do not send older documents unless requested.

B) CHART NOTES must be from an MD (medical doctor) or a DO (doctor of osteopathy). Washington patients may be able to use records from a ARNP (nurse practitioner). In other states, our doctors cannot use records from a NP or PA.

C) Any recent MRI or x-ray REPORTS (not required). Please do not send the film, just the report.

Step 2: Get your Chart notes- Tell them you are going to see a specialist for your condition. They will usually hand them over to you in a couple of minutes or a day or so. It depends on the office. You can have your doctor's office fax or mail your medical records to us directly, or bring them in yourself. In either case, we need your name and telephone number on the documents. Note: Please BE SURE to clarify with the doctor's office that you only need 2-3 visits of chart notes pertaining to your qualifying condition, NOT your entire medical record. Also ask your doctor's office to include your phone # with the fax.

Note: if you are concerned about keeping this confidential or from having the RCW 69.51a conversation with your doctor, please have them send the entire record, minus any films/x-rays.

A) Patients MUST FAX/MAIL records before making an appointment. Allow 2 business days to occur before we call you and establish an appointment time.  If the C.A.Clinics staff does not contact you within 2 business days, please call the office to ensure that the C.A. Clinics staff has in fact received your medical records.

What to Expect at Your Appointment or, The six point punch list of the Medical Marijuana recommendation Holder:

1. Check in- when you arrive for your appointment we must copy your drivers license or Washington State identification card. This is the law, we have to validate that you are a Washington State Resident.

2. Fill out a detailed three-page questionnaire if you are a new patient.

3. Pay Authorization Fee when you meet with our Clinic Manager, he/she will answer any remaining questions you have about RCW 69.51a. You will also receive information about your states laws and how to responsibly stay within the guidelines.

4. Meet the Doctor or ARNP (Advanced Registered Nurse Practictioner) The Doctor or ARNP must review your medical records with you, and determine your qualifying condition(s) based on RCW 69.51A.005. Upon final review he/she will complete all necessary state forms for your RCW 69.51a recommendation.

5. We must make copies of any documentation the state laws recommends/requires us to.

Note) We place a high significance on your privacy and leverage technology and industry standard best practice and procedures to secure all data and forms. We leverage encryption technologies as well redundant information systems to protect our customers data.

6. You get your recommendation-You will receive your originals and copies of your RCW 69.51a recommendation forms and we will return your medical records to you. We have an informational form we call the "Patient check list" form, which explains what you must do when you leave and how to renew your permit next year.

7. Authorization Database - C.A. Clinics is the only clinic in Washington State to offer our patients a means to direct Law Enforcement or other establishments to validate your recommendation on the internet. We add you to our online secured database as part of the exit interview so that any inquiries can be immediately validated as you leave our clinic.
