Three steps to Medical Marijuana Recommendation

Go to your Doctor

Step 1: Go to your Doctor-If you are a NEW PATIENT, in order to schedule an appointment you must first have:

A) CHART NOTES (The notes your doctor keeps in your medical file.) The records need to be current within the past three years and must pertain to your qualifying condition. We only need chart notes from 2-3 recent visits on your condition. These can be UP TO three years old. For example, if you saw the doctor in September of 2007, those records could be used until September of 2010. Please do not send older documents unless requested.

B) CHART NOTES must be from an MD (medical doctor) or a DO (doctor of osteopathy). Oregon and Washington patients may be able to use records from a ARNP (nurse practitioner). In other states, our doctors cannot use records from an NP or PA.

C) Any recent MRI or x-ray REPORTS (not required, but desirable if applicable). Please DO NOT send the film, just the report.

Step 2: Get your Chart notes- Tell them you are going to see a specialist for your condition. They will usually hand them over to you in a couple of minutes or a day or so, it can vary by physician. You can have your doctor's office fax or mail your medical records to us directly. In either case, make sure your name and number is on them. Note: Please BE SURE to clarify with the doctor's office that you only need 2-3 visits of chart notes pertaining to your qualifying condition, NOT your entire medical record. Also ask your doctor's office to include your phone # with the fax if you fax them.

Note: if you are concerned about keeping this confidential or from having the Medical Marijuana conversation with your doctor, please have them send the entire record, minus any films/x-rays.

Please Fax medical records to (800) 747-9275 or deliver by postal mail to:

Attn: Clinic Manager-P.O. Box 966 Port Angeles, WA 98362

Patients MUST FAX/MAIL records before making an appointment. Allow 2 business days to occur before we call you and establish an appointment time.  If the C.A.Clinics staff does not contact you within 2 business days, please call the office to ensure that the C.A. Clinics staff has in fact received your medical records.

Step 3: Call and let us know when we can expect your records to arrive and how they are being sent. Also check our schedule to determine which clinic you will be attending. We will need roughly 7 to 10 days to review your records , upon completion we will contact you. If you do not hear from us by day ten, please contact us by telephone.

It is that easy. Then we schedule your appointment and you are done.

NOTE: Our Washington clinics are generally held Tuesday thru Thursday 2nd and 4th week of the month. Call to see if we are in your county or check our schedule. Thanks from all of us at Cannabis Authorization Clinics and we look forward to seeing you soon.
