At the heart of the RCW 69.51a recommendation community in Washington State; our Authorization Clinic works with patients dealing with Chronic Pain and Illness in respect to Washington State RCW 69.51a

We hope that you will consider us as your authorization provider for RCW 69.51a recommendation. A place where patients can feel safe, comfortable and cared for.

Go to your Doctor

Step 1: Go to your Doctor-If you are a NEW PATIENT, in order to schedule an appointment to get a RCW 69.51a recommendation we must first have:

A) CHART NOTES (The notes your doctor keeps in a medical file.) The records need to be current within the past three years and must pertain to a qualifying condition. We only need chart notes from 2-3 recent visits. These can be UP TO three years old. For example, if you saw the doctor in September of 2007, those records could be used until September of 2010. Please do not send older documents unless requested.

B) CHART NOTES must be from an MD (medical doctor) or a DO (doctor of osteopathy). Washington patients may be able to use records from a ARNP (nurse practitioner). In other states, our doctors cannot use records from an NP or PA.

C) Any recent MRI or x-ray REPORTS (not required, but desirable if applicable). Please DO NOT send the film, just the report.

Call us to schedule an appointment.

Step 2: Request-Chart notes- Tell them you are going to see a specialist for your condition. They will usually hand them over in a couple of minutes or a day or so, it can vary by physician. Most doctor's office can fax or mail medical records to us directly. In either case, make sure your name and phone number is on them. Note: Please BE SURE to clarify with the doctor's office that only need 2-3 visits of chart notes pertaining to your qualifying condition are needed, NOT the entire medical record.  

Note: if there is concern about keeping this confidential or from having the RCW 69.51a discussion with a primary physician, please have them send the entire record, minus any films/x-rays.

Patients MUST FAX/MAIL records before making an appointment for your RCW 69.51a recommendation. Allow 2 business days to occur before we call and establish an appointment time.  If the C.A.Clinics staff does not call within 2 business days, please call the office to ensure that the C.A. Clinics staff has in fact received the medical records.

 Step 3: Call and let us know when we can expect the records to arrive and how they are being sent. Also check our schedule to determine which clinic you will be attending. We will need roughly 7 to 10 days to review the records , upon completion we will contact you. If you do not hear from us by day ten, please contact us by telephone.

It is that easy. Then we schedule your appointment and you are done.
